Add swap usage as a monitoring metric

Add swap usage as a monitoring metric (for linux based servers). Either number of swap pages in use, or perhaps swap rate.

    We're in the middle of a major monitoring redesign, and as a starting place for swap metrics we are planning to add a memory plot that splits out swap and rss usage.

    To Ben Y:

    We also have custom metrics support on our longer-term radar and in the near term will be releasing a new, more Prometheus node_exporter compatible metrics agent.
