Your update query for Network Access Trusted Sources is wrong. The "where" is by the "name" column, and the name is the IP address, so If you put the same one on 2 different Sources (by mistake), you can't edit it without changing all the sources with the same ip (even changing the Description) and end up having duplicated entries.
